Guide for clinics

How to reduce no-shows at a chiropractic clinic

Every no-show is a patient you already paid to bring in, sitting in a slot nobody else can use. Here is what actually brings the number down, without turning your front desk into a reminder-calling machine.

A missed appointment costs a clinic more than one visit. The adjustment slot sits empty, the chiropractor's hour is still paid for, and a patient who skips one visit in the middle of a care plan is more likely to drift away from the plan altogether.

Most clinics already send a reminder. The ones that keep no-shows low do a few more things around that reminder, and almost none of them need the front desk to pick up the phone.

1. Remind twice, at the right times

One reminder the day before is easy to read and forget. Two reminders do different jobs:

  • Two days before, so a patient who can't make it still has time to move the visit, and you still have time to fill the slot.
  • The morning of (or a couple of hours before), as a nudge for the patient who simply forgot.

Send them by text. Patients read texts; email reminders land in the same pile as everything else.

2. Ask for a reply, not just a read

A reminder that asks for nothing gets nothing back. Ask the patient to reply C to confirm or R to reschedule. Now the schedule tells you something: confirmed visits need no more attention, and only the handful of patients who didn't answer need a call. That is a short list instead of a whole afternoon of dialling.

3. Make rescheduling one tap

Patients who can't easily move an appointment often don't. They just don't come. Put a direct link to your online booking in every reminder so moving the visit takes ten seconds, and the empty slot turns into a rescheduled one rather than a no-show.

4. Have a plan for the 2 p.m. cancellation

When a patient cancels on the day, the slot is usually lost. Keep a short fill-in list of patients who want an earlier visit. When something opens up, one text goes to that list and the first person to reply gets the time. Your front desk confirms it; nobody has to work through a phone list.

5. Follow up the same day when someone doesn't show

A kind message the same afternoon (sorry we missed you, here is a link to pick a new time) gives patients who were embarrassed to call an easy way back. Waiting until someone remembers next week is how a missed visit becomes a patient who quietly stops coming.

6. State your cancellation policy early, and kindly

Patients respect a policy they were told about up front. Mention it when the visit is booked and in the confirmation message, in plain words: how much notice you need and why. You don't have to charge for every miss for the policy to work. Most of its value is that patients know the slot matters.

7. Track one number every week

Your no-show rate is the number of no-shows divided by the number of booked visits for the week. Write it down every Monday. Without it, you can't tell whether any of the changes above are working, and you can't see a bad week coming back.

Use what your booking software already does

Practice management software such as Jane, Cliniko and ChiroSoft already handles parts of this. Before adding anything new, check what yours is set up to do today: which reminders go out, when, by text or email, and whether patients can reply. Then fill only the gaps, like two-way confirmations, the fill-in list and the same-day follow-up. If you run Jane, our Jane App integration guide covers what connects and how.

One rule for every message: keep clinical details out of reminder texts. The time, the clinic and a link are enough.

Where to start this week

If you only change one thing, add the reply-to-confirm reminder two days before each visit. It gives patients time to rebook, gives you time to fill the slot, and shrinks the list of people your front desk has to chase.

This is the kind of system MoFluent builds and runs for chiropractic and physiotherapy clinics, connected to the software you already use rather than replacing it. See how we work with clinics.
